All athletes must complete the JCPS Middle School Athletics Package, which includes a Parent Permission/Release Form, Health & Safety Video Form, Student-Athlete Concussion Statement and KHSAA Physical Form. 

All forms must be uploaded to Final Forms by the parent.  The Parent Permission/Release Form MUST be notarized before it is uploaded. Please make sure both parent and student have signed the forms before uploading.
